Packages

Packages are designed to provide additional savings to your routine care. Packages are nonrefundable and expire 12 months from purchase date. Packages are non- refundable.

Memberships

Memberships are designed to provide additional savings to your routine care. Memberships are nonrefundable and require a 3 month minimum commitment, cancellable after initial 3 month commitment with a 30 day notice. Monthly membership fee will be processed thru automatic payments each month.Memberships are non- refundable.

Cancellation and No Show Policy

If you need to cancel your appointment, please cancel online, call/text our office (787) 417-7794 or email us within 24 hours of your scheduled appointment. A failure to cancel a scheduled appointment without a 24 hour notice or a "No Show" may be charged a cancellation fee of $25.00 to credit card stored on file.

Changes

We reserve the right, at our sole discretion, to modify or replace these Terms at any time. Please review our Terms and Conditions periodically to review any changes. What constitutes a material change will be determined at our sole discretion.

SMS Terms & Conditions

By opting in to receive SMS messages from Aviva Family Chiropractic, you agree to receive text messages related to your interaction with our clinic. These messages may include appointment confirmations, appointment reminders, scheduling assistance, clinic notifications, and follow-up care communication related to chiropractic care, massage therapy, and assisted stretching services.
